Your Rights as a Sunflower Clinic Client
Every person who receives care at Sunflower Clinic has rights that we are committed to protecting. You will receive a written copy of your rights when you begin services, along with a verbal explanation in language that makes sense to you, and we will ask you to sign a form confirming you received them. You can request a copy at any time.
As a Sunflower Clinic client, you have the right to:
- Be treated with dignity and respect, and receive appropriate services regardless of race, color, national origin, religion, creed, ethnicity, sex, age, mental or physical disability, marital status, sexual orientation, sexual identity, or political beliefs.
- Keep all of your civil rights. Receiving treatment never takes away rights guaranteed to you by law or the constitution.
- Participate in developing your own treatment plan, and receive an explanation of the benefits, risks, and alternatives of any treatment or medication recommended to you, including the risks of no treatment.
- Refuse treatment and be told what the consequences of refusing may be.
- Review, copy, or receive a summary of your treatment records, request corrections to information you believe is inaccurate, and add a written statement to your record if you disagree with something in it.
- Have your treatment records kept confidential, as described in our Notice of Privacy Practices and Confidentiality of Treatment Records pages.
- Review our fee schedule and receive an explanation of your bill, no matter who pays for your care.
- Receive services free from abuse or neglect of any kind.
- Be informed about any research or experimental procedure proposed as part of your care, and refuse to participate without affecting your services.
- Raise concerns, file grievances, or recommend changes to our policies or services, to our staff, to government officials, or to anyone else, without any interference and without fear of retaliation.
Never discharged for a return to use
If you experience a return to substance use, that alone is never grounds for discharge from our care. We will work with you to re-engage and update your treatment plan.
How to raise a concern or file a complaint
You may contact your clinician, or reach the clinic at 717-208-2889 or compliance@sunflowerclinic.com. For clients in Michigan, our Recipient Rights Advisor is Kera Passante, VP of Therapy (717-208-2889, compliance@sunflowerclinic.com); you may also file a rights complaint with the program, with your regional entity, or with the Michigan Department of Licensing and Regulatory Affairs (LARA). For clients in Pennsylvania, complaints may also be directed to the Department of Drug and Alcohol Programs (DDAP). Filing a complaint will never affect your care.
